Search Tips Just answer a few questions on this form and you can view: * Company's business name(s) and primary address(es). * Jurisdictions in which the company either currently provides telecommunications service or in which it plans to offer service in the next year. * Names and business addresses of various company officers. * Company's designated agent for service of process. * Names under which the company has conducted business in the past. Search Tips:

• 499 Filer ID Number: Enter complete Filer ID number if known, otherwise leave blank.
• Registration Number (CORESID): Enter complete Registration Number if known, otherwise leave blank.
• Principal Communications Type: Select type of communications services the company provides from the popup box if known, otherwise select "Any Type".
• Legal or Trade Name of Company: Enter company name if known, otherwise enter a partial name.  The search will return all results containing your entry.  Example: If you enter phone in the entry box, the results will return all companies with names containing the phrase phone. You may leave this field blank if you don't have any information about the company name.
• State or Jurisdiction: Select the state where the company provides telecommunications if known, otherwise select "Any State".  If you want to find companies that provide services in more than one state, you may select multiple states by holding down the CTRL key while selecting additional states.
• All Selected States:  If multiple states are selected, the search will return only those records where services are provided in all selected states.
• Any Selected State:  If multiple states are selected, the search will return records where services are provided in any of the selected states.
